Ah sorry, ran out of ones I could remember quick and felt like digging through episode summaries to jog my memory wasn't really in the spirit of the game!  I will see if something occurs to me by tomorrow morning, either a response or a new topic (you put me on the spot!)

I was down to weak guesses and fantasies at this point too. Like the Carmichael Twins from "Scoobynatural" and I think Dean slept with someone in "Regarding Dean", but I've only seen that episode a couple times and I'm not sure. Did we already list Carmen from "What is and What Should Never Be?" I had thought of her earlier but didn't say it because it's not "real".  Anyway, well played!

"Most vampire lore is crap." Decapitation is the classic, but Vamptonite will work if you have it.

Demon!Dean.....nothing can kill it :).  He can only be de-demon'd.

Came up with a couple more multiple-episodes-but-not-dead people: Harry Spengler and Ed Zeddmore.

 

Werewolves -- silver to the heart is the easiest way to kill it.

Wouldn't Cain have been able to kill Demon!Dean with the first blade?  Dean was able to kill him, and if I'm not mistaken Cain was a demon?

But since it didn't actually happen, sure why not :)

 

I'll go with:

Ghosts, salt and burn the corpse*

*unless corpse has already been salted and burned, in which case look for an object they are attached to**

** unless object is a donor organ, impaling the person seems to work in that case

Theorectically yes. I was thinking of more after Dean killed Cain there was no one else who could kill him.

Leviathan

You're gonna need the bone of a righteous mortal washed in the three bloods of the fallen for this one. In a pinch, you can ask them to eat themselves or get another Leviathan to do it.
