This is Cookie's saucy dig at Harper Scott, the journalist who has been working on a profile of Lucious for roughly 100 years. Along the way, "fact-checking" and "good quotes" have evolved into "sexual tension" and "Lucious obviously wants to bang her," and since Cookie doesn't like other women slithering up next to her man, she throws shade. When Lucious promises he'll send Cameron some fine adult beverages, Cookie suggests a homemade douche instead, presumably for her dirty puss. This underscores the fact that Cookie and Lucious are totally into each other again and will soon be named co-CEOs of Empire. I love every second of it. I'd like to see Cookie stand before a row of characters and read them all!

The best part of this scene is when Cookie picks up a napkin and places it over the chair cushion where Harper was seated before sitting down on it herself.  Lol!
