Diana Galbadon confirmed that was Jamie's ghost, yes. He's already had his life with Claire and died, and he looks up at her knowing that she's going to meet him soon from her perspective.

Unfortunately, if that happened then Jamie would be out of luck. He doesn't have the time travel gene and can't be sent to the future. Galbadon said very explicitly that he can't time travel "as much as it would be amusing to see Jamie Fraser try to figure out a microwave" (paraphrase).
